Patient Rights
You have the right to receive considerate and respectful care at the PMU Health Care Service and to be treated with dignity and respect, regardless of your race, religion, age, beliefs.
You have the right to an explanation of your diagnosis, treatment, and prognosis in terms you can understand.
You have the right to complete explanation of any research or experimental procedure proposed for treatment, and the opportunity to give your informed consent before any procedure is begun.
You have the right to obtain a second medical opinion prior to any procedure.
You have the right to refuse medical treatment, except as prohibited by law, and to be completely informed of the consequences of making this decision.
You have the right to involve family members in your care decisions.
You have the right to respect for your personal privacy by all staff at the PMU Student Health Care Service.
You have the right to know the names and positions of those individuals involved in your care, by official name tags or personal introductions.
You have the right to health information materials such as leaflets and brochures found in the health desk.
You have the right to express your satisfactions, suggestions and complaints in appropriate way, using the feedback form.
Patient Responsibilities
Complete the health form and return it to the Health Care Center;
Be honest about your medical history and other information you give us;
Act with consideration and respect toward PMU Health Center staff;
Be respectful of all the health care providers and staff, as well as other patients.
Ask questions if you do not understand the explanation of your diagnosis, treatment, prognosis or any instructions;
Follow health care advice and medical instructions;
Report any changes or lack of improvement in your health;
Ask for assistance when not feeling well, and to inform faculty, staff, friends or security to contact the clinic in such cases;
Respect clinic policies;
Observe the privacy and confidentiality of others in the clinic.
Excuse letters
Excuse letters must be submitted within a maximum of ten (10) days from the date of absence. The Excuse letter form should be submitted along with a detailed explanation (diagnosis) in the absence of which the Excuse Form will be considered invalid and consequently rejected.
Absences without medical report or sick leave report require a written letter or notice from the guardian along with a landline number for verification. Assessment will be made on a case by case basis.
